Good Faith Exam
Prior to scheduling your visit here at Awaken Med Spa, we require that you have a Virtual Good Faith Evaluation (GFE), also known as a medical clearance with our MD.
To ensure the safety of children and the enjoyment of all clients, we ask that parents or guardians make other arrangements for their children while they are receiving services. Children must be 14 or older to receive Spa services and must be accompanied by an adult at all times.
We accept Visa, MasterCard, American Express, and Discover. We do not accept personal checks. A credit card is required to hold all appointments.
Consultations are currently priced at $50, this amount will be added to your account and can be used on any products/services received. These consults ensure
that our provider's time is used as best as possible. Consultations last up to 30 minutes and provide the client the opportunity to have an in-depth conversation with our Nurse Injector or Esthetician to go over any questions or concerns they may have.
Virtual consultations are currently not being offered, if you have a special request or question please call 919-359-1741 to inquire.
Refunds are not provided for services or packages, but a credit can be applied toward other services. If a patient moves out of state, a refund will be provided on unused service packages.
Unopened products may be returned within 30 days for a credit to your account.
Late Arrival For Appointment
If you are late for an appointment, your treatment will end on time so that the next appointment may begin as scheduled. Should this happen, your service will be charged in full. If you are more than 15 minutes late, your appointment will need to be rescheduled.
If you are running late, please call us at 919-359-1741 or text us at 980-600-2960 so we can let your provider know.
We have a 24-hour cancellation policy for all appointments. Appointments that are canceled OR rescheduled within 24 hours of the appointment start time are subject to a $50 cancellation fee. Appointments can be canceled via Patient Portal, phone, email, or text message!
*Failure to abide by our cancellation policy may result in restricted access to appointments and services.
Gratuity is optional and can be applied at checkout through a credit card, or envelopes are provided for Cash.
Due to continued production changes and pricing increases from our vendors, we are continuing to update our medical aesthetic and product pricing. Please note that while we have put off raising prices as long as possible, we are adapting to these industry changes as best we can.
A handicap-accessible ramp is available. The spa staff is not allowed to lift clients onto tables, If such assistance is required please bring a personal care attendant.

Right to Refuse Service
We reserve the right to refuse or discontinue service to anyone demonstrating behavior that is perceived to be inappropriate or disruptive to our Spa atmosphere.
